LADO - Internal Investigation Example with Guidance

The template below is a representative example, and it is not expected that you copy it exactly. Many organisations have used it as-is with their own logos, which is perfectly acceptable. Although I will emphasise that we do ask that there is a final outcome selected from: Substantiated, False, Malicious, Unfounded, or Unsubstantiated.

Once it has been confirmed that the matter is suitable for an internal investigation, we ask that this is conducted within 10 working days. However, we understand that this may not always be possible.

We ask that the outcome report is shared with the LADO inbox (LADO@cornwall.gov.uk) for the caseworker to review, provide any feedback, and confirm the final LADO outcome.
